You will need to use semi-stretchy dental rubber bands with your braces to correct the midline . The rubber bands come in different elasticity strengths and usually are labeled with various animal pictures. The medium strength rubber band is what my dentist used for me (size 6.4mm bald eagle/American Orthodontics brand). Depending on whether you need to move the upper or the lower midline and also which direction you need to move it, you will need to place the rubber diagonally. I needed to move my upper teeth midline toward my left hand, so I wore a rubber band from the TOP RIGHT hook (close to the upper canine - the pointy tooth) down to my LOWER LEFT hook (also close to the bottom canine tooth on the left side). It may take a week before you notice the teeth moving, usually indicated by a pain at the root of the teeth and sometimes feeling itchy. The itchy pain feeling is how you know your teeth are moving. Of course, you should take these rubber bands out when you eat and put them back on after eating to have them on for the longest time. Double-banding will not quicken the movement. It could take from 2-4 weeks before it aligns with your bottom midline, depending on how much it needs to move. You should let your dentist know so he can correct his own work (that`s what you paid him for, right?) or at least ask for more rubber bands. I wanted to prevent my midline from shifting due to my braces but my dentist waited to correct mine, so I certainly understand your concern. I began asking for extra rubber bands of the kind I needed at every dentist visit so I could start correcting it on my own. This is a description of my experience, and is in no way, professional dental advice as I am not a dentist/orthodontist. Should you wish to try this, it will be at your own risk. I believe that orthodontists provide better treatment than general dentists, since they get a few more years of training especially on braces. There are a lot of one-stop shop dentists out there who may know the basics but won`t be able to give you the attention and detailed positioning that matters. A good dentist/orthodontist should be able to give a thorough stage by stage summary of what they plan to do to make sure you are comfortable with it and should also give you some pamphlets/info to take home. The dentist that keeps you informed is a good one. A dentist with a good web site that provides info and a preview of what to expect is, in my opinion, more likely a better dentist focused on the patient`s needs and comfort and not on what the "ideal bite" should be. Not everyone desires the "ideal bite," but some may just want some simple straightening. Pay attention to how your dentist answers your questions before you sign any treatment contract. If they seem to rush you, then I`d keep shopping around. Also, a good question to ask is if the dentist is the person who places the brackets on your teeth or if they let one of their assistants/dental hygienists do it. A good dentist always does it himself and doesn`t let any assistants do it. The placement and position of the brackets onto your teeth is important to how the teeth will move with the arch wire. The whyquit website on cigarette smoking cessation did a nice break down the US government`s own findings on OTC nicotine patches and their effectiveness. You can read the entire document here http://whyquit.com/whyquit/A_OTCPatch.html but the summary data indicates that on average a person has about a 11.5% chance of quitting cigarettes on their own. In a seperate experiment where the nicotine patch was used roughly the same percentage of people were able to quit. Oddly, the control group in this study where a placebo was used only produced about a 6% success rate (half that of the normal "quit on your own" study). It is from this set of studies that the pharma industries have been claiming "nicotine patches can double your chances of quitting cigarettes". You can see this is clearly a disinengyuous and self serving statement. This paper only covers OTC nicotine patches so I can`t speak about nicotine gum and it`s effectiveness.

In order to quit you need to get nicotine completely out of your system. Feeding yourself a little bit of nicotine all day is like giving a drunk an IV drip of alcohol so he won`t go drinking. It just seems like a marketing gimmick. It may reduce the "craving intensity" but you will always have a craving because you are not kicking the drug out of your body.

A paper published by J L Tang suggests 4mg gum can help "highly dependent smokers" which he defines as someone who needs a cigarette upon waking. He suggests an average of about 15% can quit using nicotine patches or gum indicating nicotine gum was more helpful than the patch. http://www.bmj.com/cgi/content/abstract/308/6920/21

The American Cancer Society (ACS) recommends using the patch for people who have just quit. They suggest starting at a higher dosage of nicotine and over time bringing this down to patches with less nicotine to "ween" yourself off of the drug. the ACS suggests nicotine replacement along with cessation counselling can double your chances of quitting cigarettes for good.

The price for ipad differs from country to country and from store to store. I don`t know how much costs an ipad in Canada, but you can ask for advice a salesman in specialised stores, or if you`re not living in Canada, you can look after specialized sites where you can find that information.
